We recognize the social, economic, and physical conditions that affect our community. Our Social Determinants of Health program draws together resources from Covenant HealthCare and our community to help patients who demonstrate a need for extra assistance. Through this program, we can assist in coordinating transportation, help with medical bills, and draw on community support programs for those in need. Patients who seek care from Covenant HealthCare Partners likely will see their out-of-pocket costs decrease and the quality of their care increase.
